- General Business 300 Course Permission Form
- Business Course Permission Forms - Students that are attempting to add a course or change a lecture or section after the Add Deadline should complete this form.
- Business Course Permission Form (PDF-Available beginning the first day of each semester). This form is used for the Fall and Spring terms
- Summer Business Course Permission Form (PDF-Available from April 15 - May 15 ). This form is used for the summer term only and for all summer sessions.

Course Change & Pass/Fail requests can be accessed through your Student Center in My UW Madison (http://my.wisc.edu) by clicking on the 'Course Change Request' link via Course Enrollment, Term Information sub tab.
You must SAVE your request and PRINT the request. If you are saving more than one request, each request will need to be printed separately. Once appropriate signatures have been obtained, you will need to submit the 'Course Change Request' to your Academic
Dean's Office for final approval.
