Instructions To Create an Account

  1. Complete the online form.

    NOTE: You must use your UW Madison e-mail account, (your e-mail address ending in @wisc.edu) or your UW Madison School of Business e-mail account, (your e-mail address ending in @bus.wisc.edu). A valid e-mail address at UW Madison or UW Madison School of Business informs the system at Qualtrics that you are authorized to create an account.
     
  2. Read the Terms of Service. If you agree to the Terms of Service, check the box labeled "I have read and agreed to the Terms of Service."
     
  3. Click the "Register" button.
     
  4. After submitting the form, you will receive a confirmation e-mail from Qualtrics. You must reply to this e-mail in order to activate your account.
     
  • Create a Survey Account Now.
     
  • NOTE: Initially, all accounts are set to the default level of use, which limits the number of survey responses to 500, the number of active surveys to 3, and total surveys (active and inactive) to 5. The default level account only gives you the minimal level of features of the Qualtrics Survey Software. Upgraded accounts must be requested to increase the number of responses for each survey and allow for more active surveys for your individual account. An upgraded account must also be requested to get all of the functions available within the Qualtrics Software. To request an upgrade, click the "Request an Upgraded Account" on the upper right of this page.
