Application Guidelines
Please submit the following application materials (8 points below) to the University of Wisconsin-Madison School of Business International Programs. FAXed or emailed documents are not acceptable. We must submit originals with appropriate signatures to the visa office.

- You must apply on-line before sending materials to our office. Click on University Special Student Application for specific instructions and the link to the application. Please use the name that is on your passport and do not use any accents, umlauts, or other such letter markings. Please note that we require a paper copy of only the APPLICATION COMPLETION INSTRUCTION SHEET, printable from this on-line application, with your materials. We do not need the entire application.
- Official University Transcript
- Your transcript should list all undergraduate and graduate courses you have taken and the grades you received
- Must have official university stamp or seal (handwritten forms and photocopies not accepted)
- Must be in English
- TOEFL Score Report
- School of Business requires a new Internet score of 100 or better, a computer-based score of 250 or better, or a paper-based score of 600 or better
- Check with your international office about this requirement
- Photocopy of official score report accepted
- Statement of Financial Support
Amount of support for 2009-2010 must equal at least $15,000/academic year or $7,500/semester. Copies or FAXes are not acceptable. Please send originals.
Statement must be one of the following:
- Bank statement in your name
- Certification
of Financial Support
signed both by your parents (or financial sponsor) and
his/her bank - If the bank will not sign the Certification of Financial Support, you can submit the Certification of Financial Support signed only by your parent (or financial sponsor) and either a bank statement in that person's name or a different letter from the bank with the bank's signature and official seal.
- If your sponsor is a corporation or university, submit an award letter from sponsor.
- A combination of the above documents
Obtain two original versions. Send one with application and keep one for when you take the IAP-66 to the United States Embassy or Consulate to apply for the visa.

Application Deadlines
Your school should nominate you by
- March 15 for fall semester (begins in September) Deadline may be extended by request.
- September 1 for spring semester (begins in January) Deadline may be extended by request.
You will then receive an email from our office inviting you to apply on-line and your documents should follow as quickly as possible.
