Tom Formolo joined CHS in 1990 and was promoted to partner in 1997. Prior to joining CHS, Formolo was employed by Heller Financial, Inc. as a vice president in the Leveraged Funding Group.  Formolo serves on the Board of Directors of AMF Bowling Worldwide, Inc., Qubica/AMF Products Worldwide SARL, KBA Holdings, Inc., Suture Express, Inc. and American Reprographics Company (NYSE: ARP).  Formolo is also the chair of the Board of Trustees of North Shore Country Day School and serves on the Board of Directors of Chicago Fellowship, Heartland Dental Care and WEB Laundry.

He received his BBA in 1986 from UW-Madison and his MM in 1991 from Northwestern.
